Scam Type Debt Collections Scam Date Reported Feb 17, 2018 Victim Postal Code 97229 Total Dollars Lost $0.00 Scam Description Caller announced they were a legit gov agency conducting a survey about health, that my number was chosen by random. They asked if I was on my phone and driving, I replied no, then assured me they would ask for no personal info that could identify me. . .and I could quit the call at anytime. . .it felt scammy and I ended it there. Immediately thereafter I received a call from 503-515-1245. . .I said hello, there was sounds. . Male-ish and I shut the call down.

Said Oregon Public Health Services. Hung up after I answered (Survey) reported by


A man said he was calling from the Oregon Public Health Services for an anonymous survey. They asked me if I was able to talk and if I was driving, so I said "no I am not", heard my voice through the other end, and then they hung up. Immediately blocked the number and cursed myself for not ignoring the number earlier.

I never answer calls from unfamiliar numbers. If they leave a voicemail and are legit then I return their call. This number has called me once every 3 or 4 days for the past month and never left a voicemail which leads me to believe it's a scam call. I'm going to try to find a way to block the number from contacting me!

I have NO idea where this number could be from, but there are too many numbers for it to originate from the USA.

I received a letter, from the "Oregon Health Authority", about a week ago, letting me know that I had been selected, to do a "Phone Survey", that would help "OHP" improve their services, to People who have "OHP" There's no need to worry, as this is the Phone number, of the Survey Group! It is an Oregon Phone number.

I got the same one but I don't answer numbers I don't know. I think scam or telemarketer either way I'm not answering or calling back. If it was important they could of left a message.
