CDC - National Immunization Surveys, The Center for Disease Control's National Immunization Surveys (NIS) are a group of phone surveys used to monitor vaccination coverage among children 19–35 months and teens 13–17 years, flu vaccinations for children 6 months–17 years, and COVID-19 vaccination for children and teens in eligible age groups and for adults 18 years and older. The surveys are sponsored and conducted by the National Center for Immunization and Respiratory Diseases (NCIRD) of the Centers for Disease Control and Prevention (CDC) and authorized by the Public Health Service Act [Sections 306]. Data collection for the first survey began in April 1994 to check vaccination coverage after measles outbreaks in the early 1990s.

Don't answer. They are NOT the CDC. They want to steal your stimulus money. They offer you $20 to finish the "survey" even if you only give month and day they can guess the year bc they specifically ask about children under age 5 or 6 I can't remember now.pretty easy to figure out the year. Please don't pick up or give your child's information and DON'T open any links they send or offer you to look them up. They are fake websites made to trick you into believing they are legit! Im so worried someone is going to trust these pll & give out information to a scammer. We are all struggling right now. This is SO WRONG! they use phone apps to change their # and even make it a local # - please PLEASE don't fall for this and if somehow you did contact the IRS IMMEDIATELY!!! Also report the numbers and warn others. I hope this helps somebody.
